Easy Access to Your Child’s Records

Need a copy of your child’s records? We are here to help. Simply complete the student records request form—available in both English and Spanish online or at the District Office reception.

How to Submit Your Request

Local Pickup in Glendale:
Bring your completed form and a valid ID (driver’s license, passport, etc.) to our District Office at 7301 N. 58th Avenue, Glendale, AZ 85301. We are open Monday through Friday, from 7:30 a.m. to 4:30 p.m. (Closed on holidays.)

Out-of-State Requests:
Email or fax your completed request form along with a copy of your photo ID, and include your phone number and mailing address.
